Terms and conditions for Unlimited Monthly Membership: 12 Month Initial Commitment
This contract is made between the client and North San Antonio FIT4MOM and requires a 12 month commitment except as provided in the initial 10 day cancellation policy below. This membership allows you to attend unlimited Stroller Strides/Stroller Barre classes and requires a 12 month commitment. This membership does not automatically end after the initial commitment. A cancellation form will need to be submitted at least 15 days before the last payment due date.
AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM North San Antonio to initiate a charge to the account entered into my account for my dues. This authorization is to remain effective until I have cancelled my membership in writing through https://app.e2ma.net/app2/audience/signup/1998409/1402783/
I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM North San Antonio to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.
CANCELLATION POLICY
To cancel this
membership, the member must fill out this form https://app.e2ma.net/app2/audience/signup/1998409/1402783/ at least 15 days prior to their next payment due
date to request a cancellation. Cancellations will be confirmed in
writing to the member once they have been processed. No refunds will be given for dues once it has been paid.
This membership is not transferable to another person or anywhere
outside the North San Antonio franchise. Once a membership is cancelled, if the
member wants to re-enroll they will be charged half the normal
registration fee. Instructors are not authorized to cancel memberships.
HOLD POLICY
Memberships
may be held a maximum of 4 months per hold period. To place a hold on
this membership, the member must fill out this form https://app.e2ma.net/app2/audience/signup/1998408/1402783/ at least 15 business days prior to
their next payment due date to request a hold be placed on their
membership. Holds will be confirmed in writing to the member once they
have been processed. Hold starts will coincide with the next date dues
is scheduled to be drawn out on and can end on any day of the month.
When a hold period is ended, the monthly auto payments will automatically
resume on the return date. It is up to the client to keep track of
their return date.
INITIAL 10-DAY CANCELLATION POLICY
You,
the buyer, may cancel this agreement at any time prior to midnight of
the tenth (10th) calendar day after you have signed this agreement. To
cancel this agreement written notice must be received by FIT4MOM North San Antonio within the initial ten days. You are responsible for paying for
the classes already attended.
AGREEMENT
By purchasing
this contract, you are agreeing to the terms of this contract. Terms of
this agreement may change with proper advance written notice.
